Insurance & Referrals
Our practice has contracts with most major insurance plans including Medicare, Humana, UnitedHealthcare, Health Net, BCBS, Cigna, TRICARE, and some AHCCCS plans. Because of our specialty, many of the HMO, AHCCCS, and other insurance plans may require a referral from your primary care physician prior to being treated by our physicians. If you have an HMO plan, we kindly ask you to verify that your referral has been sent to our office before your visit. Without a referral, our office cannot guarantee that your insurance will pay for your visit. As courtesy, we will bill your secondary insurance plans.
Co-Payments
Your health insurance coverage will be verified by our staff prior to your arrival. If you have a co-payment due, our staff will collect this at the time of service. For your convenience, we accept Mastercard, Visa, American Express, and CareCredit.
You can now use CareCredit's "Pay My Provider" feature. Be sure to search for us under "Sheftel and Associates Dermatology, LLP."
Biopsy & Test Results
If a procedure requires a biopsy, please understand that test results may take up to two weeks to return to our office. Once your test results are received from the lab, they are reviewed by your physician. Your physician will provide detailed instructions to our nursing staff regarding your results and any additional procedures, testing, and recommendations that are deemed necessary. You will receive a phone call notifying you of the test results.
Prescription Refills
When you require a prescription refill, please contact our office at least 48-hours prior to your last dose. Your refill will often require a review of your information by your physician and may require prior authorization from your health plan before a refill can be sent to your pharmacy. When requesting a refill, please call our office at (520) 293-5757 and be prepared to provide the following information:
- The patient's name
- Name and dosage of the drug that needs to be refilled
- The prescription refill number located on the prescription container
- Name and phone number of your pharmacy
